For Facial Treatments

BEFORE PROCEEDING, please review and complete the following documents as pertains to your desired appointment:

For Chemical Peels:

For all other facial treatments:

skin reading intake form

Forms can be printed and brought with you to your appointment. If you are unable to print, we will print for you and they can be filled out at the time of your appointment.

If we have intake forms already on file, proceed to scheduling.

Cancellation Policy

Glow Aesthetics implements a firm 24 hour cancellation policy. Cancellations under 24 hours or no shows will be charged 50% of the appointment scheduled. You will be unable to reschedule until the fee is paid. Thank you for your understanding.
